A Rare Case of Cytomegalovirus Colitis with Subsequent Non-tubercular Mycobacteria Immune Reconstitution Inflammatory Syndrome

چکیده

Cytomegalovirus (CMV) colitis occurs commonly in immunocompromised patients with high mortality. CMV infection has also been reported immunocompetent individuals and it a varied clinical presentation. When HIV-infected are started on antiretroviral therapy (ART) there is reconstitution of the immune system which results paradoxical worsening existing conditions or development new disease known as inflammatory syndrome (IRIS). In setting IRIS one most common infections to occur non-tubercular mycobacteria (NTM). The generally develops when CD4 count < 50 cells/µL. Here we present rare case followed by NTM IRIS, its management, treatment outcomes.

Cytomegalovirus immune reconstitution inflammatory syndrome

28 Background: Appendicitis occurs with increased frequency in HIV infected compared to 29 HIV uninfected persons. CMV-related appendicitis specifically presents with typical 30 appendicitis symptoms including surgical abdomen, fever and leukocytosis and may have 31 a more severe course with higher mortality than other types of infective appendicitis. We 32 report the first case of CMV appendic...

Nontuberculous Mycobacteria Immune Reconstitution Syndrome

The prevalence of nontuberculous mycobacteria infection (NTM) in Sub-Saharan Africa is estimated to be less than 1%. NTM is often underdiagnosed or misdiagnosed as tuberculosis in patients who present with immune reconstitution syndrome (IRS) following initiation of antiretroviral treatment (ART). Immune reconstitution syndrome is common in patients who start ART with low CD4 counts and high HI...

Immune reconstitution inflammatory syndrome.

Immune reconstitution inflammatory syndrome (IRIS) is a collection of inflammatory disorders associated with paradoxical worsening of preexisting infectious processes after the initiation of highly active antiretroviral therapy (HAART) in human immunodeficiency virus (HIV)-infected individuals. At the initial stage of the disease, the imaging findings can be misdiagnosed as pneumonia resulting ...

Coping with TB immune reconstitution inflammatory syndrome.

The TB immune reconstitution inflammatory syndrome (IRIS) is a relatively frequent complication in HIV-TB-coinfected patients after they start highly active antiretroviral therapy (HAART). There are two forms of TB IRIS: the 'paradoxical' type (clinical worsening of a patient on TB treatment) and the 'unmasking' type (undiagnosed TB becoming apparent after starting HAART). Their pathogeneses ar...

Immune Reconstitution Inflammatory Syndrome: A Reappraisal

Individuals with human immunodeficiency virus infection who commence antiretroviral therapy when they are very immunodeficient are susceptible to immune reconstitution disorders. The most common disorders are the various forms of immune restoration disease (IRD) that appear to result from the restoration of a dysregulated immune response against pathogen-specific antigens. Essentially, any path...
